There’s this expectation that once the baby arrives, everything should feel joyful, natural, and somehow fall into place. And while there are definitely beautiful moments, there’s also a whole side of postpartum that doesn’t get talked about enough.
Your body has just gone through a major physical event, your hormones are shifting rapidly, and you’re adjusting to an entirely new routine, all at the same time. It’s a lot, and it can feel overwhelming in ways you didn’t expect.
Things like postpartum anxiety, postpartum bleeding, and even figuring out basics like postpartum underwear can catch you off guard if no one prepared you for them. And the truth is, many women are left trying to figure it out as they go.
So let’s talk about what actually happens after birth and what more women deserve to know going into it.
1. Your Hormones Drop Fast
One of the biggest shifts happens almost immediately after delivery. Within 24 hours, estrogen and progesterone levels drop dramatically, and your body has to adjust quickly to that new baseline.
That sudden change can affect everything. Your mood, your sleep, your anxiety levels, and even how your body feels physically. You might feel emotional one minute and completely overwhelmed the next, and it can seem like it came out of nowhere.
This is a major reason why postpartum anxiety and mood swings are so common. Your body is undergoing a rapid hormonal reset while you’re also adjusting to a brand-new routine and level of responsibility. It’s a lot all at once, and your body is doing its best to keep up.
2. Postpartum Bleeding Lasts Longer Than You Expect

It’s called lochia, and it can last anywhere from 4 to 6 weeks as your body sheds tissue and heals internally. It usually starts as bright red, then gradually shifts to pink, and eventually brown or lighter spotting.
This is also where things like postpartum underwear become essential. You need something comfortable, absorbent, and realistic for what your body is going through. This isn’t the time for anything restrictive or uncomfortable.
Keep in mind:
- Bleeding should gradually get lighter over time
- Small clots can be normal early on
- Soaking through a pad in an hour is not normal and should be evaluated
Understanding what’s normal can help you feel more prepared and less caught off guard during recovery.
3. Your Pelvic Floor Needs Recovery Too
This is one of the most overlooked parts of postpartum healing, but it plays a huge role in how your body feels long after delivery.
Your pelvic floor supports your bladder, uterus, and bowel, and it goes through significant strain during pregnancy and birth. It doesn’t just automatically return to normal, even if everything feels “fine” at first.
You might notice:
- Pressure or heaviness
- Leaking when you laugh, cough, or sneeze
- Pain with intimacy
- A feeling of weakness or lack of control
These symptoms are common, but they do not have to become your new normal. Pelvic floor therapy or pelvic floor exercises can be incredibly helpful and can make a noticeable difference in how you feel day to day.
Taking care of this early can prevent longer-term issues and help you feel more like yourself again.
4. Sleep Deprivation Impacts Your Hormones
Everyone expects to be tired after having a baby, but the impact of sleep deprivation goes much deeper than just feeling exhausted.
When your sleep is constantly interrupted, your body struggles to regulate important hormones such as cortisol, insulin, and thyroid hormones. This can affect your energy, mood, metabolism, and overall recovery.
Sleep deprivation can also impact:
- Cortisol levels (your stress hormone)
- Blood sugar balance
- Thyroid function
- Milk supply
So if you’re feeling more anxious, emotional, or just not like yourself, it’s not just about being tired. Your body is trying to function without the rest it needs to regulate everything properly.
This is another reason why postpartum anxiety can feel more intense during this stage.
5. Postpartum Mood Changes Are Real
The “baby blues” are a common experience for many women in the first 1 to 2 weeks after delivery. This can include crying, mood swings, irritability, and feeling overwhelmed.
For many, these feelings improve as hormones begin to stabilize. But for others, those emotions don’t go away or may even intensify.
If you notice:
- Persistent sadness
- Ongoing postpartum anxiety
- Feelings of rage or irritability
- Intrusive or racing thoughts
…it’s important to reach out for support. These experiences are more common than people talk about, and they deserve attention and care.
You are not expected to just push through or handle it alone. Getting support early can make a huge difference in your recovery.
6. Postpartum Recovery Is More Than 6 Weeks
Here’s the part that needs to be said more often. Postpartum recovery is not a 6-week event, even though that’s often how it’s presented.
Your body is still healing, adjusting hormonally, and finding a new rhythm well beyond that initial checkup. This includes your physical recovery, your mental health, and your overall sense of balance.
Healing takes time, and it looks different for everyone. Giving yourself permission to move at your own pace is one of the most important parts of recovery.
