Irregular Period & PMS
A pregnancy test can't show where a pregnancy is growing. Doctor P, OB/GYN, explains hCG, false reassurance, and what finds an ectopic pregnancy.
Can An Ectopic Pregnancy Be Detected By A Pregnancy Test?
October 19, 2026
A positive pregnancy test feels like an answer. Then comes a twinge low on one side, or a little spotting you did not expect, and a new question shows up in the middle of the night. Does that test tell me this pregnancy is where it belongs?
It does not, sis, and I would rather you hear that from me now than learn it the hard way. Let me walk you through what a pregnancy test can show, what it cannot, and what actually finds an ectopic pregnancy.
Quick answer: No, a pregnancy test cannot detect an ectopic pregnancy. A home test and a blood test only show that you are pregnant, not where the pregnancy is growing. An ectopic pregnancy is found with a pelvic ultrasound, often along with repeat hCG blood tests. If you have a positive test with one-sided pain or unusual bleeding, call your doctor right away.
I'm a board-certified OB/GYN, and in this video I explain what a pregnancy test can and cannot tell you about an ectopic pregnancy, and how doctors find out where a pregnancy is.
What A Pregnancy Test Actually Measures
Before we talk about what a test misses, it helps to know what it is looking for. A pregnancy test is built to answer one question, and it answers that question well.
What hCG is and when your body makes it
hCG is human chorionic gonadotropin, the pregnancy hormone. Your body starts making it after a fertilized egg attaches to the wall of your uterus, and both urine and blood tests are designed to pick it up.
If you are curious about the timing between sex and that attachment, I cover it in how long after sex implantation happens.
Home urine test versus blood test
A home urine test is a yes or no test. It turns positive once the hCG in your urine passes that test’s threshold, often around 20 mIU/mL, and that number varies by brand.
A blood test is different because it gives an exact number, and it can pick up smaller amounts of hCG. Both tests tell you that you are pregnant. Neither one tells you where.
Why A Positive Test Can Give False Reassurance
An ectopic pregnancy is a pregnancy that grows outside the uterus, most often in a fallopian tube. It still makes hCG, so it still turns a pregnancy test positive.
A positive test only says you are pregnant
That is the part that catches people off guard. The test result looks exactly the same whether the pregnancy is in the uterus or not, so a positive result cannot rule an ectopic pregnancy out.
If you want the warning signs that should send you to a doctor, I walk through them in what the 3 signs of an ectopic pregnancy are.
What hCG does in a typical pregnancy
In a typical early pregnancy growing in the uterus, hCG climbs quickly. At first it nearly doubles every two days, and it usually rises by at least about half every two days.
What hCG can do in an ectopic pregnancy
In an ectopic pregnancy, hCG often rises more slowly or levels off. But not always. In one large study, about 1 in 8 ectopic pregnancies had hCG that rose like a typical pregnancy.
So a number that is going up is not proof that the pregnancy is in the right place. This is why doctors do not rely on a single hCG result. They look at the trend, your symptoms, and an ultrasound together.
How Doctors Find Out Where A Pregnancy Is
The test that answers the location question is a pelvic ultrasound. Your care team may also do a pelvic exam and repeat hCG blood tests, but the ultrasound is how they actually see where the pregnancy is growing.
When the ultrasound cannot see it yet
Early on, an ultrasound can come up empty even though you are pregnant. Guidelines use a deliberately conservative hCG level of about 3,500 mIU/mL for when a pregnancy should usually be visible. It is not a hard line, and a pregnancy can still be too early to see below it.
What a pregnancy of unknown location means
When you have a positive test but the ultrasound cannot find the pregnancy yet, it is called a pregnancy of unknown location. That does not mean it is ectopic.
Most turn out to be an early miscarriage or a pregnancy in the uterus, and a smaller share turn out to be ectopic. In my practice, I tell patients this is a waiting room, not a diagnosis. Repeat hCG blood tests and a repeat ultrasound are how we follow it until the answer is clear.
What Happens If It Is An Ectopic Pregnancy
An ectopic pregnancy cannot be moved to the uterus, so it always needs treatment. There are two options, and which one is right depends on whether the tube has ruptured.
Medication with methotrexate
Methotrexate is a medicine that stops the pregnancy from growing. It is only an option before the tube has ruptured, and only if you are able to get the follow-up blood tests, which are essential. Your body then absorbs the pregnancy over several weeks.
Surgery
Surgery is needed if the tube has ruptured, and sometimes even when it has not. It is usually done through small cuts with a camera, called laparoscopy, and it may mean removing the fallopian tube. A rupture is an emergency that needs surgery right away.
When To Call Your Doctor And When To Go To The ER
If you have a positive pregnancy test and pain on one side of your belly or pelvis, or bleeding that is not normal for you, call your doctor right away and ask for an ultrasound to find out where the pregnancy is.
Go to the emergency room or call 911 if you have any of these.
- Sudden, severe pain. In your belly or pelvis.
- Shoulder pain. Especially with a positive test.
- Heavy bleeding. Or bleeding that is getting worse.
- Weakness, dizziness, or fainting. These can be signs of internal bleeding.
Tell them you have a positive pregnancy test. Do not wait for a callback, and do not drive yourself if you feel faint.
Getting A Clearer Picture Of Your Own Health
A scare like this often makes people want to understand their reproductive health before the next surprise, not after it. I created the Ultimate Hormone Assessment as a starting point to help you understand your own hormonal picture instead of guessing at it.
Frequently Asked Questions
Can a pregnancy test detect an ectopic pregnancy?
No. A home test and a blood test both detect hCG, which an ectopic pregnancy also makes, so a positive result only shows that you are pregnant. A pelvic ultrasound is how your care team finds out where the pregnancy is growing.
Can you have a negative pregnancy test with an ectopic pregnancy?
It is uncommon, because an ectopic pregnancy makes hCG too. But a test taken too early can read negative in any pregnancy. If you have pelvic pain or unusual bleeding, do not let one negative test be the final word. Call your doctor.
Can hCG levels tell you if a pregnancy is ectopic?
Not by themselves. hCG that rises slowly or levels off can point toward an ectopic pregnancy, but some ectopic pregnancies rise like a typical one. Doctors look at the trend, your symptoms, and an ultrasound together.
What is a pregnancy of unknown location?
It means you have a positive pregnancy test, but the ultrasound cannot yet show where the pregnancy is. It does not mean the pregnancy is ectopic. Repeat hCG tests and a repeat ultrasound are used to follow it until the location is clear.
How is an ectopic pregnancy treated?
It always needs treatment. Methotrexate is only an option before the tube has ruptured, and you need to be able to get follow-up blood tests. A rupture is an emergency that needs surgery right away, and surgery is sometimes needed even when the tube has not ruptured.
