Understanding the Steps After a Breast Cancer Diagnosis

Every October, the world turns pink, and while the ribbons, walks, and fundraisers are wonderful reminders of solidarity, Breast Cancer Awareness Month is also about education. As an OB-GYN, I’ve walked alongside many women through one of the hardest sentences to hear: “You have breast cancer.”

I’ll be honest, no one is ever prepared for those words. They are some of the hardest to deliver for doctors. The mix of shock, fear, and “what now?” is overwhelming. But here’s the important part: there are clear steps after a breast cancer diagnosis that help guide you through what comes next. Knowing what to expect can make the journey just a little less overwhelming.

Step 1: Confirm the Diagnosis

After an abnormal mammogram or biopsy, the first step is always confirmation. Your care team may order additional imaging or pathology reviews to be sure. It’s completely normal to feel anxious during this stage, but getting accurate results sets the foundation for your treatment plan.

Step 2: Learn the Stage and Type

Not all breast cancers are the same. Your doctor will determine the “stage” (how advanced it is) and the type (for example, Ductal carcinoma,  Paget’s disease, Angiosarcoma). This helps your team create a treatment plan tailored to your body.

Step 3: Meet Your Care Team

One of the biggest things I tell patients: you won’t go through this alone. You’ll meet a team that may include a breast surgeon, medical oncologist, radiation oncologist, and sometimes a plastic surgeon. Each plays a role in your care.

Step 4: Discuss Treatment Options

Treatment depends on your stage and type of breast cancer. Options may include:

  • Surgery (lumpectomy or mastectomy)
  • Radiation therapy
  • Chemotherapy
  • Hormone or targeted therapy

Your doctors will talk through the pros and cons of each, but remember: you have a voice in this process. Asking questions, seeking a second opinion, and understanding your choices are all part of the steps after a breast cancer diagnosis.

Step 5: Consider Fertility and Hormonal Health

This is where OB-GYNs often step in more closely. Treatments like chemotherapy can affect fertility and hormone levels. If you’re thinking about having children in the future, or you’re navigating symptoms like hot flashes or changes in menstrual cycles, bring this up early. There are options to preserve fertility and manage symptoms.

Step 6: Emotional and Mental Health Support

A cancer diagnosis isn’t just physical. It takes an emotional toll. Depression and anxiety are common, and no one should face them alone. Counseling, support groups, or even just talking with other survivors can make a world of difference. October is also Depression Awareness Month, which is a good reminder to care for your mental health as much as your body.

Step 7: Plan for Follow-Up and Recovery

Even after active treatment ends, follow-up is essential. Regular imaging, lab work, and visits with your care team will be part of your life moving forward. Recovery doesn’t happen overnight, but ongoing surveillance and lifestyle changes, like nutrition, exercise, and limiting alcohol, can support long-term health.

Hearing “you have breast cancer” is life-changing, but knowing the steps after breast cancer diagnosis can give you a sense of direction, from confirming the diagnosis to building a care team, discussing treatment, and caring for your emotional health. You don’t have to face any of it alone.
