WOMEN'S HEALTH

Feeling tired or cold all the time? Learn the common signs you might be iron deficient and why it’s especially important to get tested.

Are You Iron Deficient and Don’t Know It?

October 23, 2025

As women, we’re used to powering through. We take care of kids, partners, careers, households, and somewhere along the way, we’ve learned to normalize feeling tired, run down, or just not ourselves. But what if that exhaustion isn’t just “life”? What if it’s your body trying to tell you something?

Iron deficiency is one of the most common nutritional issues worldwide, and here’s the part that often gets overlooked: Black women are twice as likely to be iron deficient compared to white women. Even more concerning, we may not even know it.

Why Iron Deficiency Matters

Iron plays a key role in carrying oxygen throughout your body. Without enough of it, your cells simply aren’t getting the fuel they need, and you end up running on empty. Think of it like trying to drive a car with just a splash of gas in the tank; it sputters, struggles, and can’t perform the way it should.

For women, iron deficiency is already common because of heavy periods, pregnancy, and dietary gaps. But for Black women, the risk is even higher and often overlooked. Part of this comes from conditions like fibroids, which are far more common among Black women and can lead to heavy, chronic blood loss month after month. Add in the fact that many of us aren’t being screened as often as we should be, and it becomes easy for iron deficiency to sneak up and stick around for years without a diagnosis.

The impact isn’t just about feeling tired; it affects heart health, brain function, and even fertility. Left unchecked, low iron can lower your quality of life in ways that people might dismiss as “just being busy” or “just stress.” That’s why it matters to bring awareness to this issue, especially in communities where women are more likely to be affected and less likely to get answers quickly.

Signs You Might Be Iron Deficient

Iron deficiency is more common than most people realize, especially among women, athletes, and those who follow plant-based diets. It’s not always dramatic or obvious at first; in fact, the symptoms can show up slowly and blend right in with everyday life. Maybe you’ve been chalking up your exhaustion to being “just busy,” or assuming your brittle nails and hair loss are from stress. But sometimes, your body is quietly trying to tell you that something’s off.

Here are some of the most common symptoms:

  • Constant fatigue – No matter how much you sleep, you feel drained.

  • Shortness of breath – Even light activity, like walking up a flight of stairs, leaves you winded.

  • Brittle nails – Nails that break easily or have ridges can be a clue.

  • Hair thinning or shedding – If your hairbrush is looking fuller than usual, iron could be the culprit.

  • Feeling cold all the time – Especially when everyone else around you seems comfortable.

Why Black Women Are More at Risk

Iron deficiency doesn’t affect everyone equally, and for Black women, the risk tends to be significantly higher. It’s not just about diet or lifestyle; it’s a mix of biological, cultural, and healthcare factors that all add up over time.

Many Black women push through fatigue, headaches, or dizziness without realizing these could be signs of something deeper. And because symptoms are often dismissed or misdiagnosed, iron deficiency can go untreated for years.

There are a few reasons Black women tend to have higher rates of iron deficiency:

  • Heavier periods – Many women of color experience heavier menstrual bleeding, which can lead to greater iron loss month after month.

  • Fibroids – Black women are more likely to have uterine fibroids, which can cause heavy bleeding and worsen iron deficiency.

  • Dietary differences – Some cultural eating patterns may naturally include less iron-rich foods, or the body may not absorb certain plant-based sources as easily.

This combination makes it even more important for Black women to monitor their iron levels, pay attention to subtle changes in their energy and mood, and advocate for proper testing when something doesn’t feel right.

Getting Tested and Taking Action

If any of these symptoms sound familiar, it’s worth asking your doctor for a simple blood test. A CBC (complete blood count) and ferritin level test can show if you’re low on iron or developing anemia. Don’t brush it off or assume you’re “just tired.”

If you are iron deficient, here are a few steps to help:

  • Add more iron-rich foods – Lean red meat, chicken, fish, spinach, beans, and lentils are great options. Pair plant-based iron with vitamin C (like citrus or bell peppers) to help your body absorb it better.

  • Consider supplementation – Sometimes food alone isn’t enough, and an iron supplement may be needed. But always check with your doctor first, since too much iron can cause other issues.

  • Manage heavy periods – If heavy bleeding is the root cause, your OB-GYN can discuss options to reduce blood loss and protect your iron levels.

Why This Matters

Living with low iron is not just about being tired. It impacts your whole life. Energy, focus, mood, and even your physical appearance can all suffer. I’ve had countless patients come back to me after treating their iron deficiency and say things like, “I didn’t realize how bad I felt until I finally felt good again.”

As women, especially as Black women, we often push through and normalize feeling run-down. But your health deserves more than that.

If you’ve been dealing with unexplained fatigue, hair shedding, or that never-ending chill, it might not be “just life.” It could be your body telling you it’s low on iron. Ask for the test, have the conversation, and don’t minimize your symptoms.

Because when you take care of your iron, you’re not just restoring your blood. You’re restoring your energy, your confidence, and your ability to show up for yourself and the people you love fully.
